Specific IgE is better than skin testing for detecting Aspergillus sensitization and allergic bronchopulmonary aspergillosis in asthma.

چکیده
We read with interest the review by Schulman et al 1 in a recent issue of CHEST (January 2015) wherein the authors suggest the use of a skin test for screening sensitization to various allergens in preference to specifi c IgE antibody testing in patients with asthma. We agree with the authors that all patients with asthma should be screened for allergic sensitization against a particular allergen depending on its prevalence. However, we do not agree with the authors’ recommendation that skin testing is the preferred approach as this can potentially miss allergic sensitization against Aspergillus fumigatus . In a study comparing the performance of a skin test (intradermal) or specifi c IgE (against A fumigatus ) for the diagnosis of allergic bronchopulmonary aspergillosis (ABPA) in 372 subjects with asthma, the sensitivity of an A fumigatus -specific IgE and Aspergillus skin test was 100% and 88% to 95%, respectively, in various models of latent class analysis. 2 Thus, if a skin test is used as a screening modality, one can potentially miss 5% to 12% cases of ABPA.

Sensitization to Aspergillus antigens and occurrence of allergic bronchopulmonary aspergillosis in patients with asthma.
BACKGROUND Allergic bronchopulmonary aspergillosis (ABPA), which is predominantly a disease of asthmatic subjects, is caused by hypersensitivity to Aspergillus antigens. Screening for Aspergillus sensitization in asthmatic subjects could identify those who are at risk for ABPA. Few studies have shown that fungal sensitization could be an important risk factor for asthma severity. We sought to d...

Bronchial asthma is an inflammatory disease of the airways which may be worsened due to numerous extrinsic factors. The most common trigger is continuous exposure to allergens of which fungal agents are important factors. There is overwhelming evidence for the presence of fungal sensitization in patients with asthma. The diagnosis of fungal sensitization can be made either with skin testing wit...

Exposure and sensitization to fungal allergens can promote the development and worsening of allergic diseases. Although numerous species of fungi have been associated with allergic diseases in the literature, the significance of fungi from the genera Alternaria, Cladosporium, Penicillium, Aspergillus, and Malassezia has been well documented. However, it should be emphasized that the contributio...

asthma, who attended for the first time to Allergy Department and collected data on clinical history, respiratory function and allergic sensitization. Asthma severity was classified using the GINA guide. We integreted the data into regression models to identify allergen sensitization most strongly associated with asthma severity. Results: We included 260 children with ages ranging from 3 to 18 ...
متن کاملUncertain areas in the diagnosis of allergic bronchopulmonary aspergillosis in patients with asthma.
BACKGROUND AND OBJECTIVE The prevalence of allergic bronchopulmonary aspergillosis (ABPA) in patients with bronchial asthma remains unknown. We evaluated the roles of various laboratory tests in the diagnosis of ABPA, including, skin prick test (SPT) for Aspergillus fumigatus (Af), and serum Af specific IgE and IgG antibody measurement. METHODS A total of 50 asthma patients with more than 100...
